Coffy, a nurse, has witnessed the negative effects of drugs firsthand, but it isn't until her younger sister develops a heroin addiction that she decides to take up arms on her own. Coffy, a killer who poses as a prostitute, starts off by killing street dealers and pimps like the sleazy King George before moving up to more powerful people. But when she learns that her boyfriend, a politician, is implicated, she is shocked.
